Eligibility & Benefits

Medicaid and CHIP program eligibility is determined by the Health and Human Services Commission. Providers should verify each patient’s eligibility for the date of service prior to services being rendered. There are several ways to do this:

Medicaid members may also go order new cards or print temporary cards online. Members may complete a 1027-A form for a replacement card.
